Go to the area where the first boss was and from there go to the room with the downward stairway (big statue in the center). The faq says it can be found after you first meet Yetka, that you let her throw the guy off the wall, then talk to her a second time and after the scene is over you should be able to loot the item for a the magic leech trinket. I just beat Lords on my first playthrough at level 76; I racked up almost 52 hours in that first playthrough because I tried to do everything and barely read anything to help me until the first time you have to go to the Rhogar dimension. Anyway, I beat Judge, obviously, and This is the first side-quest we can undertake in the game and it is broken down into several parts that span a good portion of the campaign. Upon leaving the Keystone Monastery at the start and opening the gate, you will bump into the Injured Monk immediately outside. Speaking to him will start the side-quest. Yl3e.
